Please avoid submitting multiple ads under a single order. Each advertisement must have a unique order number to be included in the souvenir journal.

Formatted Ads: Only formatted ads may be submitted/uploaded and no adjustments will be made.  These are ready to print/camera ready. They must be uploaded in  PDF, JPEG, JPG, or PNG format, and no larger than 10 MB.

Fields with an (*) are required.

The Los Angeles Chapter Charitable Trust reserves the right to modify any language and/or reject any photo and/or images deemed inappropriate.  The Cotillionette’s parent(s) will be notified regarding rejected or altered ads, so that the ad may be re-submitted.
